We often receive requests for a GP to complete a report; form or other letter for patients. These are not included in services provided by the NHS and, as such, we will charge for these.
Whilst we have a standard set of charges, each request is different and so we ask that you bring the forms to our reception who will then pass these onto our secretarial team to review. One of our secretaries will be in touch shortly afterwards to advise you of the fee for the completion of your request. Please note that we require payment prior to the work being completed and returned to yourself.
