SunnyBank Medical Centre - Information for new patients wishing to join the doctors surgery

Patient Registration

register_now_blue_sign Any new patient wishing to register with the Practice must live within the Practice catchment area (see below).

You will be asked to complete a registration form & a new patient health check questionnaire.You will also be given a new patient pack, containing information about the practice together with relevant and useful information on the NHS.

Practice Area

Any new patient wishing to register with the Practice must live in the following areas:

BD12
BD19 (Scholes only upto M62)
HX3 (Norwood Green only)
HD6 (Bailiff Bridge only)

Any patients currently registered with the Practice who change address outside the above postal codes will need to register with another GP Practice

Noticeboard

When we are Closed: the out of hours service for the practice has changed.
Between 6pm - 6.30pm call: 0345 605 9999 (local care direct)
After 6.30pm call: 111 (NHS111)

CASH Clinic: the time of the monday evening CASH clinic has now changed to between 4.30pm - 5.45pm

Warfarin Clinic - We now run a warfarin monitoring clinic, this is open to all patient on warfarin. For further information or to book an appointment please contact the surgery. .

Repeat Prescriptions will be ready for collection 2 working days after ordering from 4pm.
